GENERAL 
 
1. It shall be the Club’s overriding policy to select the best-qualified coaches (Head and 
Assistant Coaches). 
 

2. Applications:

 
(a) Applications for coaching positions shall be accepted from any interested adult (over 
18 years of age). The applications shall be on a form provided by the Club and include 
a picture of the applicant’s face (not larger than 1" x 1"). Applicants must also complete 
CYSA Form 1628 (Coaches Application Form) for District and CYSA review. 
(b) The applications shall be reviewed by a committee chaired by the appropriate 
Recreational or Competitive Program Director.  
(c) A returning coach applying for a coaching position will not be approved unless the 
coach has turned in player evaluation form(s) from his/her previous year’s team(s).
