This will help with the different procedures for the various levels of play.
MATCH PROCEDURES
Match Card Procedures for CVSC:
With CVSC INTRA-league games, collect U7 to U14 D4 CVSC game cards and then deposit the cards at the referee table (Canyon & 5Canyon).
With D4 U16 to U19 games and U9 to U19 D3 INTER-league games, winning team keeps the card. In the event of a tie, home team keeps the card.
SEND-OFF PROCEDURES:
If any players or coaches are sent off, a send-off report must be reported w/in 24 hours.

For CYSA and District IV Competitive Games:
1. Send coach/player passes and report to District IV Discipline Chair, Roger Wildenstein.
2. Contact League/Club Representative (appropriate age group coordinator) who will notify the respective Discipline Chair (CVSC Discipline Chair is Sue Smith,

3. The Discipline Chair will inform the League Representative of the suspension required after the Disciplinary Committee and Board approves the action.
4. In subsequent matches, the referee shall record that the suspension is being served.
5. U.S. Club sanction tournaments/games DO NOT count toward CYSA suspensions.
6. The coach/player pass will be returned to the League/Club Representative once the suspension is served.

For game day please follow the following procedures:
1. Home team must provide a copy of the game card or sheet printed off of www.norcalpremier.com
2. To access the game card, the manager must login under his/her ID. Click on the game number to the left of the game you are going to play. Once you have chosen this screen you will see a small link on the top left of information that says game card. Click on this and an Adobe PDF will come up with the match-up for that game.
3. This form needs to be given to the referee for the game
4. Each team must supply two US Club rosters for the game. One US Club roster will go to the referee and one US Club roster will go to the other teams coach or manager.
5. Once the game is complete, the referee will return the completed game card signed by both coaches to the home team manager.
6. The home team manager must then make a copy of this for their own records to avoid any future disputes in game scores.
7. The home team manager has two options to turn this in.

8. The managers last responsibility is to log in once again and update the score for the game online. This can only be done by the home team manager.
Rule 210: League discipline
A send off (red card) will result in a minimum of a one (1) game suspension. A send off as a result of two cautions will result in the player being suspended for the next match. All suspensions shall commence immediately. If a second game is to be played that day, the suspended player is not eligible and it should be noted, by the referee, on the game card. The home team is responsible for informing the league office that a red card infraction has occurred, and the league will inform all parties affected the length of suspension. Referees will return all player passes to the teams. They will list on the Game Card any send-offs with player name and US Soccer Club number, and infraction Referee will need to fill out a send off report and send it to NorCal. The Center Referee that had issued a Send/Off (Coach or Player) should fill out the MS Word version of the NorCal Referee Send-Off Report which can be found at NorCalpremier.com, and E-mail that directly to

For GSSL League Game Match Cards & Send-Offs (U12 & U19 Class 1 teams):
1. Prior to each match, please be sure to get a match report from each team. One report is for your records the other is to be mailed to the match secretary.
2. Prior to each match be sure to get an envelope stamped and addressed to the match secretary. You are at liberty to refuse to start the match until it has been provided.
3. Please ensure that the match report is completed following the match and that all hand-written comments and names are legible.
4. Please include telephone numbers of the referee and assistant referees on the match report.
5. Please mail match report promptly following the match.
When necessary, please write reports of players and coaches that were sent off. Mail those reports, along with match reports and player passes, to the disciplinary chairperson within 24 hours of the match. Name and address of the disciplinary chairperson is at www.goldenstatesoccer.com. As the player/coach serves the suspension, the disciplinary chairperson receives the "orange card".
